Bound in Beinecke copy are four leaves from a twelfth-century ms. on parchment (probably a breviary) with musical notation in neumes.
Rubricated throughout; capital spaces with guide-letters, capitals supplied in red or blue; book numbers in red in upper center margin of each page; headings in ms. in upper right corner of each recto.
Binding: contemporay Dülmen binding of the blind-stamped leather over wooden boards; original upper clasps (clasp-hooks wanting).
Provenance: "Liber p[ri]m[u]s decretalis D[omini] H[er]bordi Wernyngh de Borchorst pastoris in Vreden atq[ue] Rectoris p[ri]mi altar[is] eccl[es]is ... [unidentified proper name"]. "In a different hand: Quem idem donauit post morte[m] domui Castrimarie p[ro]pe Dulmania[m] Ord[inis] Carthusien[sis] cum multis aliis quare merito ... fideliter pro eodem etc." (ms. on 1 a); "W.S.
